A 53-year-old man presents to his primary care physician with a 1 month history of nausea and malaise. He has been feeling under the weather for the last 3 months and has experienced a productive cough during that time period. He became concerned after he started having waves of nausea associated with occasional vomiting. He has a history of hyperlipidemia and hypertension for which he takes lisinopril and atorvastatin. He also has a history of depression that has been well-controlled on fluoxetine. He drinks 5-8 alcoholic beverages per week and has smoked 1 pack per day since the age of 20. His temperature is 98.7°F (37°C), blood pressure is 131/83 mmHg, pulse is 84/min, and respirations are 12/min. Physical examination does not demonstrate any significant findings.
