A 54-year-old woman presents to her primary care physician with a 4 month history of diffuse body pain. Her pain is located in specific points on her posterior head and neck, upper arms, back, and chest. Over the past few months, she has also had insomnia and felt increasingly fatigued. She denies fever, chills, vision changes, stiffness, or temporal scalp tenderness. Her medical problems consist of hypertension, hyperlipidemia, and hypothyroidism. Her medications include amlodipine, rosuvastatin, and levothyroxine. She has never smoked cigarettes, drinks 1-2 alcoholic beverages per week, and denies illicit drug use. A pain diagram she completed is shown in Figure A. Her temperature is 99.4°F (37.4°C), blood pressure is 120/70 mmHg, pulse is 80/min, and respirations are 16/min.
