A 72-year-old man presents to the clinic for an annual well-check. The patient endorses generalized weakness and an unintentional 10-lbs. weight loss over the past year. He also complains of fatigue and tiredness that is worse at night. He denies shortness of breath, chest pain, dark urine, abnormal stools, syncope, or nausea. His past medical history includes hypertension, diabetes, and chronic obstructive pulmonary disease. The patient takes lisinopril, metformin, and a budesonide/formoterol inhaler and is compliant with his medications. He smoked for 25 years (quit 4 years ago) and drinks 3-4 beers a day. He is currently retired and used to work at a pipe manufacturing plant. Physical exam demonstrates a thin man with mild scleral icterus and diffuse abdominal pain with palpation.
