A 44-year-old man with a past medical history of diabetes presents to the emergency department with severe knee pain. He states that he has had this pain before but never to this extent. He is unable to ambulate on the knee secondary to the pain and nothing he has taken helps with the pain. Yesterday, he states he drank more alcohol than usual for a celebration but otherwise can recall no other events or trauma to the knee. Review of systems is only notable for abdominal pain, which has been previously diagnosed as peptic ulcer disease. His temperature is 98.7°F (37.1°C), blood pressure is 159/92 mmHg, pulse is 83/min, respirations are 15/min, and oxygen saturation is 99% on room air.
