A 58-year-old African-American male presents to the emergency department with shortness of breath and cough for 5 days. He says that he started noticing the symptoms after flying across the country 1 week ago. He reports that he has recently traveled to Ohio to see his family and has two sick contacts at home. He denies having any recent hospitalizations. His past medical history is significant for HIV (CD4: 142 cells/µL), hypertension, hyperlipidemia, coronary artery disease, osteoarthritis, and G6PD deficiency. His home medications include lisinopril, atorvastatin, metformin, and aspirin. He is not currently on any medications for his HIV. His temperature is 101.6°F (38.7°C), blood pressure is 121/73 mmHg, pulse is 106/min, respirations are 34/min, and SpO2 is 88%. On physical exam, he appears uncomfortable and dyspneic with coarse rales heard on lung exam. An arterial blood gas is drawn which shows the following:
