A 58-year-old woman presents to her primary care physician with complaints of an aching pain and stiffness in her neck, shoulders, and hips for the past several months. She reports difficulty in rising from a seated position as well as in raising her arms above her head. The patient also states that she has had fatigue and chronic fevers for the past month. Close and careful physical examination reveals normal muscle strength (despite some pain with testing and palpation), but limited range of motion of the neck, shoulders, and hips.
