He also endorses that the aliens are responsible for his fathers death and that the aliens implanted a chip in his head to constantly speak to him. Auditory hallucination, paranoia, disorganized speech, and delusions are classic presentation for schizophrenia. Atypical antipsychotics, such as quetiapine, are ideal for long-term therapy as they have fewer extrapyramidal symptoms when compared to typical antipsychotics.

Diagnosis of schizophrenia requires two or more of the following for longer than 1-month period: delusion, hallucination, disorganized speech, catatonia, and negative symptoms. These symptoms must last for more than 6 months. Symptoms lasting less than 6 months but more than 1 month is called schizophreniform disorder. Typical or atypical antipsychotics are used as first line therapy with both drug classes having significant side effect profiles. Psychotherapy is not known to be an effective mode of treatment and overall prognosis even with pharmacological intervention is not favorable.

Holder et al. reports that schizophrenia is seen in under 1% of the world population with more male patients than female. While it is seen in adolescents, it is more often presents itself in the late 20s. While atypical antipsychotics have fewer extrapyramidal symptoms, care providers must monitor for metabolic changes in patients.

Javitt et al. reports that while earlier research focused of schizophrenia’s effect on brain’s higher processing function, recent findings have pointed towards early processing deficits diffused throughout the cortex. Interestingly, these deficits in cortex have been in areas associated with auditory function.
