This patient's signs and symptoms are concerning for a foreign body aspiration (FBA). Definitive therapy is rigid bronchoscopy.

Rigid bronchoscopy is the definitive method for both diagnosis and treatment of suspected FBA. It allows for the discovery of a foreign body, examination of bronchial anatomy, and collection of specimens for culture. In patients with FBA, bronchoscopy must be performed in order to prevent acute airway obstruction, lung abscess, bronchiectasis, and pneumonia.

Incorrect Answers:
Answer 1: CXR is an important first step in management for the detection of a foreign body; bronchoscopy is the only intervention which is both diagnostic and therapeutic.
Answer 3: Inhaled bronchodilators and oral corticosteroids would be appropriate treatment for asthma.
Answer 4: Flexible bronchoscopy is not the preferred management in pediatric patients. The smaller lumen size impairs ventilation and object removal.
Answer 5: Antibiotics may be appropriate as adjunct therapy in a FBA in order to prevent secondary bacterial infection.
