Varenicline (a nicotinic receptor partial agonist) is a smoking cessation agent that can cause/worsen depression or suicidal ideation. Varenicline is a highly effective medication for smoking cessation. It is a partial agonist of the nicotine receptor thus mitigating/eliminating the reward response one receives when smoking. Nicotine replacement therapy can increase the efficacy of varenicline. Side effects of varenicline can include headache, nausea, and insomnia. Varenicline is also associated with depression and suicidal ideation; thus, patients with a history of depression should be closely monitored. Incorrect Answers: Answer 1: Impaired kidney function is not a side effect of this medication. If a patient has impaired kidney function, they may need the dose to be lowered. Answer 2: Seizure is not a common side effect of this medication; however, bupropion is an antidepressant that is commonly used to treat depression and aid in smoking cessation which also lowers the seizure threshold. Answer 3: Sexual dysfunction is common with SSRIs and is a major reason for non-compliance with these medications. It is not a common side effect of varenicline. Answer 5: Tachycardia, anxiety, irritability, or premature ventricular contractions can be seen if someone abuses nicotine replacement therapy which is an optimal part of the treatment regimen for smoking cessation. It is not a common side effect of varenicline. Bullet Summary: Varenicline is a smoking cessation agent that can cause depression/suicidal ideation as a possible side effect.
